Pull gross production, adjustments, net production, and collections side by side for the same month so you can see whether the leak starts with write-downs or after the charge is posted. Run an aging A/R report and separate claims still moving normally from insurance delays, patient balances, and accounts already past due. Fix one leak at a time — fee-schedule pressure, patient-financing follow-up, or payer lag — instead of treating every collections miss like the same problem.
